Abstract

Colistin remains an important last-resort therapy for multidrug-resistant Gram-negative infections, but pharmacokinetic data in critically ill patients outside of high-resource settings are limited. This prospective, observational study characterized the pharmacokinetics of the pro-drug colistimethate sodium and its active form, colistin, in 24 critically ill adult patients (half admitted with burns) at a South African critical care unit, using non-compartmental analysis in Phoenix WinNonlin. Pharmacokinetic parameters following a 9-million-unit loading dose were comparable to previously published data from other populations, and colistin AUC was negatively correlated with white cell count and estimated glomerular filtration rate. Probability of target attainment was acceptable against Acinetobacter baumannii with MICs below 1 mg/L. The authors plan to build on these findings with a population pharmacokinetic model to better individualize colistin dosing in this population.
