Abstract
Pembrolizumab dosing is based on population pharmacokinetic data largely from Western clinical trials, with limited data available for Japanese patients or those with poor clinical conditions such as cachexia. This study analyzed 270 blood samples from 76 Japanese non-small cell lung cancer patients to build a population pharmacokinetic model using Phoenix NLME, examining the relationship between clearance changes, treatment response, and survival. Median clearance was about half that reported in Western data, and patients whose clearance decreased early in treatment showed significantly better treatment response and survival than those with increasing clearance, even among patients with cachexia. Early changes in pembrolizumab clearance may serve as a predictive and prognostic marker of treatment benefit, pending further prospective validation.
